I tested it too. Here are my remarks:

Step 1:

People may not understand what 'status' means (the explanation only 
appears when clicking the option). It might be better to have it 
(*Please check "Regular" if you do NOT have a voucher.*) appear before 
selecting the option?

Step 2:


      Membership LTI?

Please write Membership LT-Innovate (not all may be familiar with the 
abbreviation).

The rest seems to be fine.
